Tally is a form building platform.
Every form seems to be identified by a public unique id, of which i've seen some examples:

- gD9k8d (unpublished)

Technical details
Tally seems to have two ways to reference a form:
This one is the simplest, shows the form in the browser
Allows being embedded to websites as a iframe.
Apparently you can also use custom domains with it.
Url Parameters
I've seen the following URL parameters after Tally urls:
